EMPLOYMENT CHECKLIST FOR RECREATION INSTRUCTOR
Thank you for considering employment with the Mechanicsburg Area Parks and Recreation Department, which is part of the Mechanicsburg Area School District organization. The following items are required of all school district employees.
1. | CRIMINAL HISTORY CLEARANCE – The cost for this clearance is $22.00. When completing this online clearance application, please select “Employment” as the reason for the clearance request. The clearance must be dated within 365 days of hire date. The District may not accept clearances for employment purposes if they were obtained as “Volunteer.” | |

CHILD ABUSE HISTORY CLEARANCE – The cost for this clearance is $13.00. You will first create an individual account and then login to complete your request. When completing this clearance application, please select “School Employee Governed by Public School Code” as the reason for the request. The clearance must be dated within 365 days of hire date. The District may not accept clearances for employment purposes if they were obtained as “Volunteer.” |

FBI CLEARANCE FORM - The cost for this clearance is $25.25. When completing this online application, please enter 1KG6XN as your Service Code. You will pre-register online and then pay (no cash or personal checks) once you get fingerprinted at the IdentoGO store, 6483 Carlisle Pike Suite 104 Mechanicsburg PA 17050. Bring your confirmation sheet and your form of identification that you choose during your pre-registration process. The clearance must be dated within 365 days of hire date. |

5. | ACT 168 FORM: see attached form– Please complete the first part and Section 1 of the Act 168 form for each current employers and for any past employers if you worked for a school or with children in these jobs. You may have several forms to complete based on your employment history. If you have no applicable employment, please check box “no applicable employment and complete and sign Section 1. | |
6. | I-9 FORM – This is a federal form concerning eligibility to work in the United States. It will be completed in person at Human Resources. For this form you will need to bring form(s) of ID. Please see the attached the list of acceptable forms of ID. | |

TB TEST – form is attached - this must be obtained before employment begins and be within 3 months of coming into the district. |

9. | PAYROLL FORMS (W-4, RESIDENCY FORM, EMPLOYEE TRACKING, DIRECT DEPOSIT) – 4 forms attached | |
10. | SAFE SCHOOLS TRAINING – Pennsylvania Act 126 and Act 71 and school district policy require employees to complete Safe Schools online video training concerning child safety/mandatory reporting and youth suicide: awareness and prevention. All district employees must complete this training, regardless of whether they work directly with children in the course of their employment. You will receive login information from Human Resources at meeting. |
