Thank you for considering employment with the Mechanicsburg Area Parks and Recreation Department, which is part of the Mechanicsburg Area School District organization.  The following items are required of all school district employees, which include the Recreation Department employees.


_____ 1. CRIMINAL HISTORY CLEARANCE -  The cost of this clearance is $8.00.  When completing this online clearance application, please select "Employment" as the reason for the clearance request.  The District may not accept clearances for employment purposes if they were obtained as "Volunteer."
_____ 2.  CHILD ABUSE HISTORY CLEARANCE - The cost for this clearance is $8.00.  You will first create an individual account and then login to complete your request.  When completing this clearance application, please select "School Employee Governed by Public School Code" as the reason for the request.  The District may not accept clearances for employment purposes if they were obtained as "Volunteer."

FBI CLEARANCE FORM - The cost for this clearance is $24.25.  When completing this online application, please choose "School District" as the reason for the request.  You will register and pay online and then get fingerprinted at the UPS Store, 4900 Carlisle Pike, Mechanicsburg, PA 17050.  Bring your 16-digit registration ID number and photo ID with you when you go to be fingerprinted.  
_____ 4. PDE 6004 FORM - Act 24 Arrest/Conviction Report and Certification Form.

ACT 168 FORM - Please complete page 1/section 1 of the Act 168 form for each current employer and for any past employers if you worked for a school or with children in these prior jobs.  You may have several forms to complete based on your employment history.  If you have no applicable employment, please check box "no applicable employment" and complete and sight Section 1.  
_____ 6. I-9 FORM - This is a federal form concerning eligibility to work in the United States.  It will be completed in person at Human Resources.

TINE TEST VERIFICATION (TB TEST) - This must be obtained before employment begins.  Please speak with Human Resources about specifics.

_____ 11.  SAFE SCHOOLS TRAINING - Pennsylvania Act 126 and Act 71 and school district policy require employees to complete approximately 4 hours of Safe Schools online video training concerning child safety/mandatory reporting and youth suicide: awareness and prevention.  All district employees must complete this training regardless of whether they work directly with children in the course of their employment.  You will receive login information from Human Resources.
