For Individuals Listed in Sapphire as a Parent/Guardian of a MASD Student:
If you did not receive the test phone call, email, and/or text message and your contact information is correct in the Sapphire Parent Portal, please complete this notification form. NOTE: Please give our system until at least 8 p.m. to send the test messages before completing the form.

For Non-Parents/Guardians:
If you are not a parent or guardian of a MASD student but received a phone call, email, and/or text message in error, please complete this form to make us aware.
